Referrals to our staff supported milieu homes, foster homes and/or private school may be made through this website (see below), by email or by fax. The referral information will be reviewed by Bayfield's Operations Manager and/or our Director of Operations, clinical staff and School Principal as appropriate. Although we can respond to most referrals within a day, the referring agency will be notified in writing of whether the referral is appropriate or not within seven (7) days of receiving the information package. Admissions are not prioritized on a first come first serve basis. The fit of the youth referred with the current clientele in the home and the youth's needs are the primary considerations when coordinating admissions.

​​The referral package should include:

  • Family and social history
  • Pertinent medical, psychological and psychiatric data
  • Placement history and present living arrangements
  • The reason for the referral
  • School related information and education history
  • The tentative discharge plan (when and where to)

Exclusionary Criteria:

  • Pregnant adolescents​
  • Medically fragile children/adolescents
  • Children/youth currently prescribed Clozapine
  • Individuals over 18 year of age
  • Severely mentally disabled youth
  • Children that have not been toilet trained
In making a decision to accept a referral, a thorough screening process is completed. This includes a review of all available information (referral package), discussions with the referral source and, if necessary, a pre-placement visit with the youth either at Bayfield or in his/her current placement to ensure suitability. When a referral is accepted for admission to a Bayfield program, a date of admission is coordinated immediately or the referral is placed on the wait list for the next available bed. When a referral is found to be ineligible for service the referring source is notified of the rationale for the rejection (e.g. age, need, risk level, etc). Where possible, alternate placement options are discussed.

In your email, please indicate that you are requesting full access to our Referrals page and identify the agency/organization that you work for. In addition to having access to our on-line Referral Form this will give you open access to the following documents:
  • Updates on our bed availability
  • Bayfield's brochure
  • Updates on developments at Bayfield
  • Current Ministry of Children Community and Social Services licenses
  • Ministry of Children Community and Social Services per diem letter
  • Proof of insurance
  • Our current Strategic Plan
  • An on-line Stakeholder Satisfaction Form
Once you have been provided a password, you will be able to access the above information and/or make a referral by selecting the Make A Referral button at the bottom of any of our website's pages. When you select this button, it will prompt you for your your password.

Password requests will only be considered from applicants with an email address from a corporate email account for a potential referring agency (i.e. CAS, CMH, government, hospital, etc.) - passwords will not be sent to personal email addresses (gmail, hotmail, etc.).
